California ID

Work Permit

In order to get a work permit,

  1. Visit your school office and ask for a work permit

  2. Fill out the form they give you

  3. Have the document signed by your parent/guardian

  4. Have the document signed by Adele Langworthy (Executive Director)

  5. Take the completed form back to your school office

  6. When they sign it, bring it back to Rising TIDE
